In addition to these basic services, our caregivers are required to document all services provided in a Daily Care Log. That’s because we feel it’s important to keep the family informed of the client’s care plan and answer any questions. The Daily Care Logs is a anchor of our caregivers services. We want everyone to feel comfortable with the care giving situation and keep the lines of communication open. Whether you live in the same town or are in a different state. Heart2Heart Homecare will make sure our Nursing Assistants/ Sitters keep in touch with the family on a recurring basis regarding our services.

Companion Care & Personal Care Services

A  companion is any employee that has at least one year experience of working with the elderly as a companion. A companion is also known as a "sitter."

Companion Care Services:

  • Limited assistance (usually stand by) with bathing or activities of daily living. (ADL)
  • Meal preparations
  • Assist with feeding
  • Making beds, changing linens, some laundry & light housekeeping
  • Medication reminders
  • Light exercise
  • Assist to doctor visits, shopping and errand services etc.

Total Respite Care -

Requires a Certified Nursing Asst./ Certified Medical Asst.


A CNA or CMA is any employee that has met certain standards of competences by the state. A CNA/CMA directly assists to nurses or Home Health Agency reporting updates on the clients status.

Certified Nursing Asst./ Medical Asst. job description:

  • Checking and recording temperature, blood pressure and pulse (vital signs)
  • Assist with ambulation, transfer and/or position changes
  • Assist with or feed when apical
  • Assist with all bathing, grooming and toileting. (ADL)
  • Meal preparations
  • Making beds, changing linens, some laundry & light housekeeping
  • Medication reminders
  • Light exercise
  • Assist to Doctor visits, shopping and errand services etc.

Couples Care: When there's more than one family member who needs assistance.
